Changing your contact details

To update details like your name, address, phone numbers or email address:

• fill out an online admin request form via: Contact us about your request - Accurx Patient Portal
• phone or visit the surgery during opening hours 

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

• Check you are in our catchment area

You will need to show us identification before we can change your phone number or email address.

You will need to show us proof of identification before we can make any changes to your person details on your medical records. 

If you have proof of the changes, such as a deed poll certificate, it is handy to be able to show us this too.
